How do I make a survey?
A survey is a form with several questions, usually a mix of choice and rating questions. Build it the same way you build a form, then read the results as charts.
- 1Create a new form and give it a clear title, for example Customer Feedback.
- 2Add a mix of question types: multiple choice for options, linear scale or star rating for opinions, and long text for open comments.
- 3Keep it short so more people finish it, and put the easy questions first.
- 4Share the survey by link, QR code or WhatsApp.
- 5Open the Summary tab to see automatic charts for each question as responses come in.
lightbulbTip: Rating and linear-scale questions are the quickest way to measure how people feel.
quiz
How do I build a quiz?
Turn on quiz mode while building, set the correct answer and points for each question, and share it. You build the quiz in SurveyHeart; people take it and see the leaderboard on Quizzory.
- 1Create a new form and switch it to quiz mode.
- 2Add your questions and mark the correct answer for each one.
- 3Set how many points each correct answer is worth.
- 4Turn on the answer sheet or leaderboard if you want people to see their score.
- 5Share the quiz link. Taking the quiz and the leaderboard run on quizzory.in.
lightbulbTip: For graded quizzes and leaderboards, send people to quizzory.in. For plain data collection, a normal form is better.
share
How do I share a form?
Every form has a share link. You can copy the link, show a QR code, send it on WhatsApp or SMS, email it, or embed it on your website.
- 1Open your form and tap Share.
- 2Copy the link to paste anywhere, or show the QR code for people to scan.
- 3Use the WhatsApp, SMS or email options to send the form straight to people.
- 4To put the form on your own website, copy the embed code and paste it into your page.
lightbulbTip: A QR code works well on printed posters, classroom screens and event desks.

- 3Use search or filters to find specific answers.

lightbulbTip: Export to Excel or CSV when you want to sort and analyse, and PDF when you want a clean copy to share or print.

Which question type should I use?
SurveyHeart has 22 question types. Pick the one that matches the answer you need, so the question is easy to answer and the data is easy to read.
- 1Use short text or long text for open answers like names or comments.
- 2Use multiple choice or dropdown when there is one correct option from a list.
- 3Use checkboxes when people can pick more than one option.
- 4Use star rating, smiley or linear scale to measure opinions and satisfaction.
- 5Use date, time, number or email for specific data, and file upload to collect documents.
lightbulbTip: Fewer open-text questions means faster forms and cleaner data to read later.
