How do I create a free online form?

Open SurveyHeart, start a new form, add your questions, then share the link. It is free and you do not need to install anything to build on the web.

  1. 1Go to surveyheart.com or open the SurveyHeart app and choose Create.
  2. 2Give your form a title and a short description so people know what it is for.
  3. 3Add questions one by one and pick a question type for each (short text, multiple choice, rating and more).
  4. 4Mark any questions that must be answered as required.
  5. 5Open Settings to set a theme, a welcome screen or a thank-you message if you want.
  6. 6Tap Share to get a link, QR code or WhatsApp message, and send it out.

How do I make a survey?

A survey is a form with several questions, usually a mix of choice and rating questions. Build it the same way you build a form, then read the results as charts.

  1. 1Create a new form and give it a clear title, for example Customer Feedback.
  2. 2Add a mix of question types: multiple choice for options, linear scale or star rating for opinions, and long text for open comments.
  3. 3Keep it short so more people finish it, and put the easy questions first.
  4. 4Share the survey by link, QR code or WhatsApp.
  5. 5Open the Summary tab to see automatic charts for each question as responses come in.

How do I build a quiz?

Turn on quiz mode while building, set the correct answer and points for each question, and share it. You build the quiz in SurveyHeart; people take it and see the leaderboard on Quizzory.

  1. 1Create a new form and switch it to quiz mode.
  2. 2Add your questions and mark the correct answer for each one.
  3. 3Set how many points each correct answer is worth.
  4. 4Turn on the answer sheet or leaderboard if you want people to see their score.
  5. 5Share the quiz link. Taking the quiz and the leaderboard run on quizzory.in.
lightbulbTip: For graded quizzes and leaderboards, send people to quizzory.in. For plain data collection, a normal form is better.

How do I share a form?

Every form has a share link. You can copy the link, show a QR code, send it on WhatsApp or SMS, email it, or embed it on your website.

  1. 1Open your form and tap Share.
  2. 2Copy the link to paste anywhere, or show the QR code for people to scan.
  3. 3Use the WhatsApp, SMS or email options to send the form straight to people.
  4. 4To put the form on your own website, copy the embed code and paste it into your page.

How do I collect and export responses?

Responses arrive in real time. View them as charts or one by one, then download everything as an Excel, CSV and PDF file.

  1. 1Open your form and go to the Responses or Summary tab.
  2. 2See the overall picture as charts, or switch to individual view to read each response.
  3. 3Use search or filters to find specific answers.
  4. 4Tap Export and choose Excel, CSV and PDF to download your data.

Which question type should I use?

SurveyHeart has 22 question types. Pick the one that matches the answer you need, so the question is easy to answer and the data is easy to read.

  2. 2Use multiple choice or dropdown when there is one correct option from a list.
  3. 3Use checkboxes when people can pick more than one option.
